Serving the Community: Care and Services at Lyon Park Clinic

Our mission at the West End Community Foundation is to serve our local residents with valuable resources via The Community Family Life & Recreation Center, we are proud to offer a variety of enrichment activities, programs and services at little to no cost.

As many local families face difficult financial situations and often don’t have medical insurance, the Lyon Park Clinic serves all members of the family and offers them a place to get the primary health care that they may need as well as access to health education. Located within the Community Family Life and Recreation Center at Lyon Park,  the Clinic is easily accessible to any Durham resident needing care. 

The Lyon Park Clinic, a joint program of Lincoln Community Health Center and Duke Community Health, was designed with the help of multiple community members to meet the health needs of residents in the South Central Durham community.

Photo May 16, 8 39 11 PMThe Lyon Park Clinic staff includes a family physician, physician assistants, nurses, nurse practitioner and an office manager. Services include:

  • sports physicals
  • preventive testing and counseling
  • well-baby check-ups
  • hearing, vision, dental, and nutrition screenings
  • appointments for acute illnesses
  • routine immunizations
  • some lab testing
  • general health education
  • mental health assessment
  • management of chronic health conditions
  • referrals to specialists

Lyon Park accepts insurance, including Medicaid, Health Choice and Medicare. For patients without insurance, fees are based on income and family size applying the sliding scale used by Lincoln Community Health Center. After-hours and backup care are provided by Lincoln Community Health Center.
